How to use a custom form on bookings and view the answers in the appointment
Edit the calendar under Settings › Calendars › Forms & Payments and pick a form built in Sites › Forms to ask customers extra questions. Then, in Calendars, open the appointment and click the Forms tab to read the answers without leaving the booking.

Before you start
- First build the form in Sites › Forms with the questions you want (reason for the visit, preferences, etc.). Include the name, email and phone fields.
- Pick a form theme that matches your booking page colors.
- The Forms tab only appears on appointments booked with a custom form; the default form (name, email, phone) doesn't show there because that data is already in the contact section.
Where to find it
Calendars › (open the appointment) › Forms
Step by step
Attach the form to the calendar
- Open Settings › Calendars, click the calendar's three dots and choose Edit.
- Open Forms & Payments.
- Under Form, swap the default form for your custom form.
- Save. From now on, anyone booking through the link answers your questions.
View the answers on the appointment
- Open Calendars and click the appointment (it also works from the contact record in Contacts or the conversation in Conversations).
- In the appointment window, click the Forms tab.
- Read the answers, laid out with the same structure as the form.
Tips
- Use the answers to prepare for the meeting: reason, history, preferences, all visible before you start.
- Answers are read-only here. To fix a detail, edit it on the contact record or in the form submissions under Sites › Forms.
- The default form's Additional Information field shows in the appointment's notes area, not on the Forms tab.
Common problems
- The Forms tab isn't there → The appointment was booked with the default form, or it was created before the custom form was attached to the calendar. Only appointments with a custom form show the tab.
- The answers are incomplete → The customer skipped optional fields. Mark the essential fields as required on the form.
- The form's colors don't match the page → The form background follows the widget background, but the fields follow the form's theme. Adjust the theme under Sites › Forms.
